To submit a claim, the first thing you have to do is make sure you have a finalized invoice and detailed medical records from your vet visit.
An invoice is an itemized bill from your vet that shows proof of payment. It confirms the services and products you were charged for during the vet visit tied to your claim. (The respective costs should appear next to each item.) The invoice must show a zero balance or that it was paid in full for us to accept it.
Medical records are a report of your pet’s yearly wellness checkup, including exam notes, lab results, and any medical information. Your vet might refer to these records as “SOAP notes.” They are not the same thing as the records of treatment tied to your invoice.
